In order to open a bank account, you must submit an application and certain documents. One of them will be a card with sample signatures of the manager and responsible persons who have the right to put them on payment and other documents, as well as a seal impression.

The card is issued in accordance with the requirements of the Central Bank of Russia. The intrabank document will remain valid until the individual entrepreneur or organization closes its account (current, deposit) with this bank or the service agreement expires.

In another case, the head of the enterprise or another responsible person may change; naturally, when changing signatures, it is necessary to change the card with samples. Every time a bank client receives payment order or a request to transfer money from the account using such and such details, the responsible employee is obliged to compare the data on the card with those presented on the payment paper or in in electronic format.

IN last years Individual entrepreneurs and enterprises make payments using the “Client-Bank” system, i.e. remotely via the Internet. When making payments, clients use electronic signatures and seals, but information on paper must still be stored in the bank.

The form of the card and the procedure for filling it out are the same for everyone. The only way to change the content is to fill in more or fewer lines about the responsible persons who have signatory authority for a specific bank account.

The bank may require the client to provide one or more copies of this card, which will have to be presented when opening the account(s). The client does not pay any fee to the bank for issuing this card, but the services of a notary certifying the form are paid separately.

There is a concept of first and second signature.

The first one must belong to:

  • to the manager, if this is a legal entity;
  • individual entrepreneur personally;
  • a person who has the right to act on his behalf on the basis of a power of attorney.

The right of second signature usually belongs to Chief Accountant or a deputy, another responsible person who, according to the power of attorney, is temporarily entrusted with signing documents. Therefore, every time there are changes in personnel, the individual entrepreneur or enterprise must submit new information to the bank. To provide their sample signatures, responsible persons come to the bank in person.

Document overview

The card form according to the All-Russian Classifier of Management Documentation (OKUD) has the number 0401026. This form was approved by Central Bank Instruction No. 28-I (09.14.2006, last amended in 2016). The client is allowed to purchase a ready-made printed form to fill out, take it from the bank or compile it on the computer himself. In any case, the form should not differ from that stated in the Instructions.

The bank can provide the client with an internal form of the form as a sample for filling out a card with sample signatures and seal imprints in 2019. But it also should not differ from that approved by the Central Bank.

Each form for such a card must contain all the necessary details for filling out. In fact, Central Bank Instruction No. 153-I, which states how to open and close bank accounts for deposits, states that any version of the form can be used.

Thus, in the unified form there is no data about the enterprise or individual entrepreneur, such as checkpoint and tax identification number, OKVED codes and OKPO, another important information. If the bank decides to provide its clients with personally designed cards, it has the right, in addition to the necessary information about sample signatures, to include fields for filling out others.

One client can have several bank accounts. If all accounts are serviced by one operator, then the client only needs to submit one original card with samples to the bank. But provided that the list of persons authorized to sign for these accounts will be the same.

If the accounts are serviced by different operators, then each will have to present such a card. The bank can also request only the original card and make copies itself. In another case, the client may be required to bring one or more originals with copies. This information must be clarified from the bank itself.

If an authorized person has the right of first or second signature, then you will also have to submit a power of attorney in the name of this employee along with the card. For example, the manager and chief accountant do not always have the right of first and second signature; sometimes the company entrusts other persons with signing documents.

The manager can keep the accounting records himself; this is what entrepreneurs most often do. Then he will have the right of first and second signature. In the second signature field you will have to indicate that it does not belong to anyone.

Any client can present a temporary card to the bank. Its difference from the permanent one is that on the front side in the upper right corner the bank employee will put the mark “temporary”.

According to the Instructions, such a card can be issued only in certain cases when:

  • the organization transfers the right to sign the main person for some time, for example, during a vacation or business trip, to another person in charge;
  • the client needs to use a temporary additional print instead of a permanent seal; the reason may be loss of the seal or damage;
  • other.

Main functions of the card

The main purpose of the sample card is quite simple:

  • To carry out transactions from a current account, a bank employee must make sure that it is carried out by an enterprise or individual entrepreneur, or rather, its representative - a person who has the right to sign on the main documents.
  • A payment document is an important order for the bank from the client. If the latter has a seal, he is obliged to place it on such a document next to the signatures of the responsible persons. To make a payment, the operator must also make sure that the client who owns the account wants to make the payment.
  • The operator must take precautions when accepting payments on paper or electronically, issuing transfers, etc., to prevent the use of fraudulent schemes. He is obliged to accept the payment document, then compare not only sample signatures, but also the client’s seal, before making a transaction on any account. Unless he sees a 100% match, according to internal bank instructions, he has every right to refuse the client to use funds.

But there are some exceptions. So, for example, starting from April 2015, small enterprises such as LLCs and JSCs, as well as individual entrepreneurs, received, under Law No. 82, the opportunity to operate without round seals. They need to spell out this provision in internal regulations. If the bank client does not have a seal, but together with the sample signature card, he submits a copy of his regulatory document to the bank.

Detailed filling algorithm

The card is filled out by 3 responsible persons:

When an employee financial institution certifies the client's signature, he puts on the card:

  • your position and full name;
  • signs;
  • indicates the date the form was issued;
  • puts a stamp intended for these purposes.

To fill in the fields, you can use a ready-made card form and make entries manually with a black or blue ballpoint pen. In another case, you can enter the data, except signatures, on a computer, and then print the form. In the future, if there are changes at the enterprise, the client provides a new card or a temporary one. If, according to the law, the bank account number changes, the bank has the right to put its mark on the card without involving the client.

An illustrative example with points

The order of how to fill out a card with samples point by point is indicated in Appendix No. 2 to Instruction No. 153-I:

  1. In the “Account Owner” column, you must enter the full name of the legal entity according to the state registration certificate. If the form is issued for an individual entrepreneur, then his full name, date of birth, and general passport details are entered in this field. Lawyers and notaries engaged in private practice must indicate the type of activity.
  2. Regarding the client’s location, you must indicate the current legal address for enterprises or registration for individuals.
  3. To contact the client, he must leave his phone number where you can contact him.
  4. In the “Bank” field you will need to indicate the name of the institution or its division where the client opens an account.
  5. The card has a place for bank notes and others; the client does not make entries there.

  7. The “Position” column indicates the position of the persons who are granted the right to sign at the enterprise. If the card is filled out for an individual (IP), then his position does not need to be indicated.

  12. If you need to indicate the terms of authority of the responsible person vested with the right to sign, then you will have to enter this period from such and such to such and such a date in accordance with the submitted administrative act drawn up by the client.
  13. The sample print is entered in the appropriate field.

Notarization of samples of signatures and seals is done at the request of the client; it is quite enough if they are certified only by a bank employee.

When opening a current account by a legal entity, a certain set of documents is generated. One of the main papers in this case is a card of sample signatures and seal impressions. It is drawn up according to standard form 0401026, approved by instruction No. 28-I “On opening and closing bank accounts.”

What is the document for?

The need to issue such a card is dictated by the security of the service. To carry out settlement and cash transactions, a bank employee is required to verify the client’s identity. In the case of a legal entity, in addition to providing a passport of a representative from the enterprise, a comparison of the signatures of authorized persons of this company is required. At the same time, the stamps affixed to the documents are checked.

The document is filled out when opening a current account or opening a deposit for legal entities and individual entrepreneurs, judicial and law enforcement agencies, and notary employees. Individuals are required to provide only a card with a signature.

In addition, the form is provided to the financial institution when:

  • loss of a previous document;
  • replacement or loss of the company seal;
  • amendments to the full name person included in the previous card;
  • changing the legal form of the company or its name;
  • temporary suspension or termination of powers by persons holding managerial positions.

Filling rules

>The document consists of two pages. On the first sheet, after the number and name of the form, the following information follows:

  1. Account owner's name.
  2. The location of the owner, that is, his legal address - for enterprises or place of residence - for individuals.
  3. Phone number.
  4. Bank's name.
  5. Bank mark. Here the information is entered by an employee of the financial institution; the client does not need to fill out this section.

On back side The following information is entered:

  1. Abbreviated company name.
  2. FULL NAME. and sample signatures of two authorized employees entitled to endorse official documentation.
  3. The term of office of those whose visas are entered. The field is filled in if this period is limited.
  4. Date of completion.
  5. Sample print. It should be clear and easy to read.
  6. Client's signature. The visa of the owner of the current account is entered here. Manager - for enterprises.
  7. Certification inscription. This field is filled in by the bank employee receiving the form. With the inscription he certifies the authenticity of the information entered in the document.

If the individual entrepreneur does not have a seal, then the field with the imprint remains empty. If the account is opened for a foreign company, a notarized translation of the card will be required. This form is kept by the financial institution.

All transactions with the current account from the moment of its opening, maintenance and closure are carried out responsible person m, as a rule, this is the director. The form of the card is the same for all credit institutions and approved by Bank of Russia Instruction No. 28-I dated September 14, 2006, in Appendix No. 1, its OKUD number is 0401026.

The card form can be printed (made) either by the client himself or by the bank. In this case, it is imperative to take into account that the form strictly corresponds to that approved by the Bank of Russia. It is only allowed to change the number of lines in such fields as “Signature sample”, “Last name, first name, patronymic”, “Account owner”, depending on how many people will have the right of first and second signature on bank documents.

In addition, provided that persons authorized to sign documents at the bank and one operator maintains several accounts with the client at the same time, then in the sample signature card column “Bank account number” can also have the required number of lines.

The card with sample signatures can be filled out either by hand (with a ballpoint pen) or using computer technology; in this case, you must use only a purple, black or of blue color. Sample signatures can only be affixed by the person responsible; the use of facsimiles in this case is not allowed.

The bank may request a sample signature card either in one copy or in several, depending on their internal requirements. Some banks require all cards in the form of originals, while others themselves can make the number of copies they need from the original.

The right of first signature of documents, as a rule, belongs to the director of the organization or a third party who is authorized to maintain accounting records on the basis of administrative documents. Most often, signatures are placed on when making various payments.

The right of second signature is often granted to the chief accountant or also to a third party who, in accordance with administrative documents, maintains accounting.

It is possible that the right of first and second signature is granted simultaneously to several employees of the organization. So the founders can reserve such rights for themselves and it is not necessary that such rights will be transferred to the chief accountant or director.

In addition, only the first signature can be indicated on the signature card - if the director independently maintains (is responsible for) accounting and manages the enterprise at the same time. In this case, an entry should be made in the “Second signature” field that the right to put a second signature on documents does not belong to anyone.

Filling out the document is as follows:

  • Signatures on the card are placed only in the presence of the person certifying this document. They can be approached by a bank representative or a notary (in this case, he is provided with the same package of documents as for a bank to open a current account).
  • The card indicates the position, surname and initials of the employee who certifies the signatures on the document.
  • The date is indicated in numbers.
  • Next, the bank employee puts his signature and puts the seal (stamp) of the bank, which is reserved for these purposes by the credit institution.

If you are opening a current account for your individual entrepreneur or enterprise, then most likely a card with sample signatures and seals will become an integral attribute of this procedure. Why is a card needed?

The purpose of the card with sample signatures and seal imprint is quite simple and clear: it serves to ensure that employees of a banking institution clearly know what it should look like:

1. Original handwritten signature of the current account owner or his representative.

2. Company seal stamp.

After all, banks are special organizations who work with the financial resources of their clients. It is extremely important for them to take all possible precautions when accepting, issuing and/or transferring them in order to exclude various abuses and fraudulent schemes.

In this regard, there are increased requirements for signatures and seals: the main thing is that they must coincide one to one with those given in bank card with samples of signatures and seal imprints, be clear and legible. Otherwise, the transaction on the account will be refused.

The bank employee may ask the client to sign or re-stamp the form. And if the signature does not 100% match the one indicated on the signature card, the credit institution has the right to refuse to provide the requested banking service.

Mandatory stamp on the card

By the way, the form of the card with sample signatures does not necessarily contain a sample of the company seal. The fact is that from April 7, 2015, organizations can refuse to use round seals. They acquired this right after the Law of April 6, 2015 No. 82-FZ came into force. From this date, LLCs and JSCs (with some exceptions) have the right to independently decide whether to have a seal or not (prescribed in the internal regulatory documents of the enterprise).

Previously, all domestic legal entities were required to have their own corporate seal (stamp). And businessmen, as now, can choose whether to conduct business with a seal or not.

One account - one card?

You can limit yourself to filling out one card with samples of signature and seal if the company opens more than one current account within the same credit institution. Required condition in this case: access to the accounts is issued to the same persons.

And vice versa: you will need to issue several cards with signatures and a seal when the company opens current accounts in different banks. For example, due to more favorable tariffs (conditions) in another credit organization.

Who fills out the card

The document in question consists of 3 parts:

  • the first is filled out by the general director of the enterprise or his representative, specifically authorized to do so by a power of attorney;
  • the second (certifying signature certification) - a notary or bank employee;
  • the third is a bank employee.

Card form

Let us say right away that there is no generally required sample signature card for a bank. At the same time, Instruction of the Central Bank of the Russian Federation dated May 30, 2014 No. 153-I “On opening and closing bank accounts, deposit accounts, deposit accounts” (hereinafter referred to as Central Bank Instruction No. 153-I) suggests using one of two options:

1. Form No. 0401026 according to OKUD (OK 011-93) from Appendix 1 to the specified Instructions:

2. Or a form for a card of sample signatures and a seal imprint according to the internal form of a bank or other organization (it necessarily includes all the details of form No. 0401026 according to OKUD (see above)).

As you can see, the card in form No. 0401026 includes only 2 signature samples and does not include such details as TIN, KPP, OKPO, OKVED codes, etc. That is, it contains a narrowly limited amount of information.

Filling out the card

For 2018, a sample of filling out a card with sample signatures looks something like this:

The rules for its design are as follows:

  • can be filled out on a PC (if the bank is ready to issue it in electronic format) or with a ballpoint pen in blue, purple or black ink (not in pencil!);
  • facsimile signature is not acceptable;
  • There should be no blots, inaccuracies, or corrections (the damaged form must be destroyed).

Each bank determines how many completed copies of the card are needed at its own discretion.

The most important thing: either a notary or a bank employee must be present when signing.

Temporary card

There is also a temporary signature sample card, which is attached to the permanent card. Externally, its only difference is that in the upper right corner on the front side there is a “Temporary” mark.

According to clause 7.13 of Central Bank Instruction No. 153-I, such a card is issued:

1. If the right to sign is temporarily transferred to persons not indicated on the original card.

2. In case of temporary use of an additional seal impression.

What is the card for?

For the document there is standard form 0401026. It is approved by banking instruction No. 153-I. However, it is acceptable to use the form that you have set internal rules specific bank.

Why do you need a card with sample signatures and seal impressions? To obtain a current account:

  • for current transactions for an individual;
  • for the needs of individual entrepreneurs;
  • for the needs of a legal entity;
  • to open a deposit.

Courts, divisions bailiffs, notaries, law enforcement agencies– all these organizations also need current accounts to operate. Therefore, you will need a card with sample signatures and seal impressions.

Forms produced by a printing house, as well as those printed independently on a printer, are acceptable for use. The card is issued to the client at the bank or can be obtained independently (for example, downloaded from the Internet from the website of a financial institution).

Special cases

A card with sample signatures and seal impressions is provided to the bank not only when opening a current account. There are a number of other cases:

  • replacing one of the signatures;
  • seal replacement;
  • loss of seal;
  • changes made to full name the person recorded on the card;
  • changes in the name of the organization;
  • changes in the organizational and legal form of the organization;
  • suspension of powers of a management body;
  • termination of powers of the management body.

A sample signature and seal on the card is necessary so that the bank employee has the opportunity to reconcile with the details specified in the payment document. If discrepancies are identified, the bank will not accept the document. Movements on the current account will not be possible.

Card with sample signatures and seal impressions: example of filling

Let's consider all the fields of the document sequentially. The “Account Owner” field must be filled out in strict accordance with the company’s constituent documents. If an account is opened for a branch or separate division, then the name must be indicated separated by a comma after the main name.

If the client is an individual, then the full name is entered. and date of birth. If the client is an individual entrepreneur, then after indicating his full name. and the date of birth must be written: “individual entrepreneur”. If we're talking about about a person employed in private practice, then after full name. and date of birth, you must enter your occupation (for example, lawyer).

Field "Location". The organization enters its legal address here, individuals and individual entrepreneurs indicate the place of registration in accordance with their passport.

In the “Phone number” field, you can specify several numbers to contact representatives of the organization.

The "Bank" field must contain the full name financial organization in which the current account is opened.

The “Bank Mark” field is filled in by authorized employees of the credit institution; the client does not have to write anything down here.

Let's move on to designing the reverse side. Field "Abbreviated name". It is indicated only if it exists. Otherwise, the full name is repeated. Individuals here duplicate the information that was entered in the “Account Owner” field.

The line “Account number” is also filled in by a bank employee after the current account is opened.

In the “Name” field enter the persons who are authorized to sign documents. It could be one person, it could be more.

You must sign in the “Sample signature” field. Signatures on bank documents will be compared with this sample in the future, so it must be one that you can easily repeat in the future, otherwise difficulties will arise.

The “Term of office” field is not filled in in all cases; you must follow the instructions of the bank employee.

Item “Date of completion”. You need to enter the date when the card with sample signatures and seal impressions was issued.

Field “Client signature” - the document is certified by the personal signature of the account owner.

"Print sample". You need to put a stamp in this place. The print must be perfectly clear and neat.

To do this, the print must be well saturated with ink, but not leave blots. It is better to practice on a separate sheet of paper. After making sure that everything is fine, put a stamp in the field without violating its boundaries. If fragments of the stamp are poorly visible on the print, the card will have to be issued again.

What to do if an organization operates without a seal?

Individual entrepreneurs have the right to work without a seal, unlike legal entities. Individuals who open deposits and accounts for personal needs also do not have a seal. But the document is called “a card with samples of signatures and seal impressions.” What should I do?

It's simple. The instructions for filling out indicate that individual clients have the right not to fill out this field. If an individual entrepreneur has a seal, then its imprint must be affixed to the card.

How to confirm the authority of persons

The card with sample signatures and seal impressions (form 0401026) must be filled out by an authorized person.

What documents are required for confirmation?

Everyone is required to provide a passport. Individual entrepreneurs also provide registration certificates, legal entities - a set of statutory documents.

In some cases, notarization of the signature of the responsible person may be required.

If we are talking about a foreign organization, then a notarized translation of documents into Russian will be required.

Where is the card stored?
